FEES & PAYMENT OPTIONS
ASSESSMENT FEES
Assessment prices vary based on the needs of your child. Please contact our office to receive an individual quote. In accordance with the No Surprises Act, we will provide you with a “Good Faith Estimate” explaining the cost of your child's assessment.

A 10% non-refundable deposit is required to reserve your child's assessment date(s).

INSURANCE
We are an out of network provider. We will provide you with a Superbill at the end of the assessment process, which you can submit for potential reimbursement through your insurance company. Reimbursement rates vary by insurance company and plan. We recommend contacting your insurance company to find out your potential reimbursement rate. Not sure where to start? FAMILY EMPOWERMENT SCHOLARSHIP
Dr. Greenfield is a named vendor through the Family Empowerment Scholarship. We accept FES funds for therapy and consultation services; however, the FES does not currently allow funds to be used for assessment fees.

We accept payment by cash, check, ACH bank transfer, and major credit cards.
